We are Agape Care Agency Ltd,  in order to provide care and offer support to you we will need to collect  and use certain personal information about you. 

Data that we collect:

GDPR defines ‘personal data’ as being any type of information that includes an individual’s:

  • Any physical identifiers

When you enquire about or service, and whilst we are your care provider, we will collect the information (sensitive data) that you provide us: 


  • your home address,  date of birth, name and contact details (including your telephone number, email address) and emergency contacts (i.e. name, relationship and home and mobile numbers)
  • your likes, dislikes and lifestyle preferences (including your religious beliefs or other beliefs of a similar nature, racial or ethnic origin, genetics, health, and sexuality (so far as they relate to providing you with suitable care)
  • Any particular allergies and any medical you may have, physical or mental conditions and in particular your care needs, this will also be found in your care plan.
  • credit or direct debit details (if you pay for some or all of our services using one of these methods)


Information that we might collect from third parties:

When  we become your care provider, there are some information that  we will need to collect from third parties such as family members, friends, next of kins, and other healthcare professionals. This information will be for the purpose of creating your care plan, and for our general record to ensure we can provide efficient and safe care. 


Information such as:  

  • your home address,  date of birth, name and contact details (including your telephone number, email address) and emergency contacts (i.e. name, relationship and home and mobile numbers)
  • your likes, dislikes and lifestyle preferences (including your religious beliefs or other beliefs of a similar nature, racial or ethnic origin, genetics, health, and sexuality (so far as they relate to providing you with suitable care)

  • any needs assessment from other health care professionals. 


How will we use your information?

We will use your information in order to provide the best care possible. We will use it: 


  • To create your bespoke care plan: this will entail all information pertaining to your needs and how they will be met, this will be shared with other health care professionals where necessary, as well as the staff members that will be taking care of you. 
  • To create invoices so that payments can be made
  • To be able to communicate with other healthcare professionals about your health or your care package
  • To communicate with family members, next of kins, friends in charge of your affairs. 
  • For quality control measures. 


Who will we share your information with?

During the course of us providing your care, there will be certain occasions where we will need to use your information to communicate with other healthcare professionals, for example your GP, chiropodists, pharmacy and others as well as other organisations, family members. This is to enable us to provide the care that is well suited to your needs. 


Additionally, we will share your information law enforcements if it is required by law. As well as any safeguarding bodies to ensure your safety. 


At no point will your information be sold, or shared with any third party.

How long will we hold your information?

We will hold your information as required by law,  for 6 years. 


Personal information will be held for 6 years. 

Complaints, compliments and concerns held for 6 years

All Accounting information will be held for 7 years 

Record of accident and near misses for 3 years
